November 10, 2025, has been officially recognized as Proactive Care Coordination Day in Alberta by the Minister of Primary and Preventative Health Services.
To recognize this day, which honours the dedicated proactive care coordination assistants (PCCAs) across the province who work tirelessly to bridge care gaps and improve continuity of care for Albertans, the Edmonton Southside Primary Care Network is planning a special online celebration on November 10.’
