In Clinic Health Coach (LPN) – Camrose Primary Care Network (Smith Clinic)
The Camrose Primary Care Network (PCN) is recruiting a Licensed Practical Nurse (LPN) to join its team as an In Clinic Health Coach. This part-time, term position (0.5 FTE) is based at the Smith Clinic in Camrose and will run from November 1, 2025, to March 31, 2027. The schedule consists of weekdays only, alternating between two and three days per week on a biweekly rotation.

The In Clinic Health Coach plays an integral role in supporting the delivery of primary care services within a family practice clinic setting. Working collaboratively with family physicians and other healthcare professionals, the Health Coach provides patient-centered care, health teaching, and chronic disease management.
Key Responsibilities:

Deliver health teaching related to healthy lifestyles, risk reduction, and preventative self-care.

Provide assessment, education, care planning, and follow-up for patients with chronic illnesses such as diabetes, COPD, hypertension, dyslipidemia, and obesity.

Support antepartum care in alignment with SOGC guidelines, including prenatal record initiation, depression screening, and coordination of investigations.

Assist with clinical tasks such as procedures, dressing changes, injections, venipuncture, pap smears, and other services as needed.

Participate in proactive patient screening and prevention initiatives guided by the Alberta Screening and Prevention (ASaP) Program.

Contribute to data management and panel management activities, utilizing the EMR to monitor outcomes and support PCN reporting requirements.
Qualifications:

Completion of an accredited Licensed Practical Nursing program.

Current registration and good standing with the College of Licensed Practical Nurses of Alberta (CLPNA).

Minimum of three years of nursing experience in a complex primary care or community setting (preferred).

Current Basic Cardiac Life Support (BCLS) certification for healthcare providers.

Ability to work to full scope of practice while demonstrating flexibility, critical thinking, and collaboration.

Valid driver’s license, access to reliable transportation, and a recent criminal record check.

Ability to manage a varied caseload and apply health change behavior methodologies.
Compensation:
Remuneration is based on education and experience, ranging from $26.45 to $34.63 per hour.
